Life & AD&D Insurance (Policy Series GRTP-4/99)

Boston Mutual offers a comprehensive portfolio of both True Group and Voluntary Term Life programs. Our policy is unique as it allows us to offer a combined program in one single policy providing ease of sale and administration for our producers and policyholders. Boston Mutual’s Group Life and AD&D programs have been designed to offer quality benefits with a variety of options that provide financial support to protect the future of employees’ loved ones.

Standard Life Provisions:

  • Accommodating Benefit Schedules – Salary Based; Length of Service; Occupation; Flat Dollar.
  • Dependent Coverage – available for both Spouse and Children.
  • Waiver of Premium – life insurance is continued without premium payments for employees who become
    totally disabled.
  • Accelerated Death Benefit – available to insured employees with a life expectancy of 12 months or less
    allowing them to collect a percentage of their life insurance benefit early. The remaining benefit is then
    payable to the beneficiary.
  • Portability – offers insured employees and their dependents the opportunity to continue their life insurance
    upon leaving employment. Boston Mutual bills for premium directly to the former employees.
  • Conversion – allows insured employees and their dependents to convert their term life insurance to a whole
    life policy without evidence of insurability within 31 days of termination from employment.
  • Bereavement Counseling – provided by our counseling partner Health Management Systems of America offers
    toll-free counseling services to all beneficiaries.

Standard Accidental Death & Dismemberment Provisions:

  • 24 Hour Coverage.
  • Education Benefit – payable in the event of an insured employees’ accidental death. This benefit provides
    each qualified dependent a percentage of the employee’s life insurance for up to four years.
  • Seat Belt Benefit – an additional 50% of the AD&D benefit is payable if the insured’s death is the result of
    an automobile accident while the insured was wearing a properly secured seat belt.
  • Repatriation of Remains Benefit – payable up to $5,000 to cover expenses occurred to return the body if the
    insured’s accidental death occurs 100 miles outside of his/her primary residence.

Short Term Disability Insurance (Policy Series GDP100)

Boston Mutual offers a comprehensive portfolio of employer funded and employee paid Short Term Disability programs, also available with an integrated Long Term program. Our integrated policy provides both Short and Long Term Disability coverage through a single policy providing ease of sale and administration for our producers and policyholders. Our disability programs are designed to protect employees against disabilities resulting from accident, physical disease or injury, pregnancy, mental disorder, etc.

Standard Short Term Life Insurance Provisions
We offer several options, including but not limited to the following:

  • Accommodating Benefit Schedules – 50%, 60%, 662/3% and 70% of weekly earnings to maximums up to $2500.
  • Flexible Benefit Durations – up to 26 weeks.
  • Definition of Disability – the insured is disabled if unable to perform some or all of the duties of his/her regular occupation with a minimum of 20% loss in earnings.
  • Elimination Periods – benefit waiting periods from 0 to 30 days.
  • Unlimited Return to Work Benefit Incentive – partially disabled insureds working and earning between 20% and 80% of earnings are eligible for up to 100% of the maximum weekly benefit, subject to standard claim adjudication.
  • On-Line Work-Life Solutions – provided by Health Management Systems of America this resource offers direct web access to life balance issues such as health and wellness, behavioral health, child and elderly care, financial calculators, legal consults, articles and video presentations.

Long Term Disability Insurance (Policy Series GDP100)

Boston Mutual offers a comprehensive portfolio of True Group and Voluntary Long Term Disability programs. Our integrated policy provides both Short and Long Term Disability coverage through a single policy providing ease of sale and administration for our producers and policyholders. Our disability programs are designed to protect employees against disabilities resulting from accident, physical disease or injury, pregnancy, mental disorder, etc.

We offer several options, including but not limited to the following:

  • Accommodating Benefit Schedules – 40%, 50%, 60%, 662/3% and 70% of monthly earnings to maximums up to $15,000.
  • Flexible Benefit Durations – Reducing Benefit Durations; 2 to 5 Years; Social Security Normal Retirement Age. Definition of Disability – the insured is disabled if unable to perform some or all of the duties of his/her regular occupation with a minimum of 20% loss in earnings subject to the program option of 1, 2, 3, or 5 Year Own Occupation; Extended Own Occupation; 2 Year Own Occupation/ADL.
  • Elimination Periods – from 30 to 360 days.
  • Return to Work Benefit Incentive – partially disabled insureds working and earning between 20% and 80% of earnings are eligible for up to 100% of the maximum weekly benefit, subject to standard claim adjudication and program options of 12 or 24 Months; Unlimited.
  • Workplace Modification – employer may qualify for reimbursement for costs associated with job site adjustments to accommodate an employee’s returning to or remaining at work.
  • Vocational Rehabilitation Services – disabled employee receiving payment may be eligible for these services based on the individual’s disability, education, skills, motivation to return to work etc.
  • On-Line Work-Life Solutions – provided by Health Management Systems of America this resource offers direct web access to life balance issues such as health and wellness, behavioral heath, child and elderly care, financial calculators, legal consults, articles and video presentations.
