Tracking Your Healthcare.gov Enrollment

Important Information If You Enrolled in a Healthcare Plan Through www.healthcare.gov

If you have enrolled in a health insurance plan through www.healthcare.gov and used our agent information during your enrollment, please read the following:

The only way we have to track your enrollment and make sure your information has transferred to your chosen insurance carrier, is to call at least 72 business hours after you completed your enrollment through the healthcare.gov website.

For us to be able to do this on your behalf, please submit the following information to us, using the secure ShareFile link below:

  • Name, Social Security # and date of birth of primary insured on policy
  • Name, Social Security # and date of birth of all dependents to be insured on policy (if applicable)
  • Healthcare.gov Application ID #
  •  Chosen plan with monthly premium and subsidy (if applicable) amount
  • Date your enrollment was completed

REMEMBER!!!
If you need your health coverage to begin on January 1, 2014, you must have your enrollment completed and submitted to www.healthcare.gov by Monday, December 26, 2013.
